- Medical care covered at 100% with $10.00 co-payment for doctor's visits

- Deductible of $150.00 per person, $300.00 per family per calendar year
- After deductible is met, plan pays 80% of covered medical expenses
- Prescription drug card available
- Pays $5.00 for generic drugs and $20.00 for non-generic drugs

DENTAL (Mutual of Omaha) |
- Our dental plan enables employees to choose any dentist they prefer
- Calendar year deductible: $25.00 per person, $75.00 per family
- 100% covered for preventative treatment/deductible waived
- 85% covered for basic treatment
- 50% covered for major treatment
- Maximum benefits are $1500.00 per calendar year for employee and each dependent
- Orthodontic coverage will be available for eligible employees and their dependents
- Reimbursement will be at 100%, with a lifetime limit of $2,500 per person.

VISION |
- Coverage through Vision Service Plan (VSP)
- Deductible of only $35.00 ($25.00 for lens/frame; $10.00 for exam)
- Plan pays 100% of normal services for panel providers to include:
- Eye examination (once each 12 months)
- Lenses or contacts (once each 12 months)
- Frames (once each 24 months)

EMPLOYEE ASSISTANCE PLAN (EAP) |

- Coverage through United Behavioral Health
- Confidential assistance for any employee and their dependents with referrals for personal, family, marital, financial, and legal problems. Up to (3) EAP face to face assessments per calendar year. Then when referred, sessions 1-10 paid with a $10.00 co-pay; sessions 11-20 paid with a $20.00 co-pay.

PENSION PLAN |
- DSCA pays the full cost of this plan
- Must complete one year of service
- Benefits based upon years of service and final average earnings over the last five years of employment
- 100% vested after 7 years of service

401(k) PLAN |
- You may contribute from 1% - 50% of your total pay on a before-tax basis up to government limit
- DSCA will match one dollar for each before-tax dollar you contribute up to 4% of your pay
- Your contributions and earnings will be 100% vested
- Any matching contributions will be vested 100% after 5 years
- 401(k) contributions from previous employer can be rolled over as of your hire date

LIFE AND AD&D INSURANCE |
- Provided to all DSCA employees
- Equal to the amount of your annual earnings, up to a maximum of $50,000
- Additional Voluntary Life Insurance is available through payroll deductions

TRAVEL ACCIDENT INSURANCE |
- Protects you while traveling on any business
- $250,000 per employee (for employees only)

VACATION |
- Full-time regular employees are eligible for paid vacation after 3 months of employment, on a pro-rated basis
- 80 paid vacation hours per year (pro-rated)
- After 5 years of employment you receive 120 hours paid vacation
- After 15 years of employment you receive 160 hours paid vacation

PAID TIME OFF |
- Full-time regular employees are eligible for paid time off after 3 months of employment
- Each employee will accrue four hours of PTO each month (48 hrs. max.)
- Earned, but unused PTO at the end of each year will be paid out, rolled over or a combination in January

HOLIDAYS |
- DSCA observes 12-14 paid holidays throughout the year
- DSCA is shut down from Christmas to New Years
|

EDUCATIONAL REIMBURSEMENT |
- Must be an approved technical/educational course or degree.
- Must have satisfactory performance for reimbursement.
- Will reimburse you 100% of the costs for tuition, textbooks, lab fees, entrance exams, enrollment fees, required supplies, ASB fees and parking up to the maximum of $2,500.00 per calendar year.
